Lets Respect Evaluation


An evaluation of the use of the Let's Respect Resources

An important part of developing the Lets Respect campaign has been the consultation with and involvement of a wide range of front line clinicians, older people and their carers, commissioners and educationalist and other professionals, drawn from health, social care, care homes and 3rd sector.
 
It is important and essential that if we are to develop this work and reach out to as many people as possible that we have honest feedback about what we are doing and about the resource including the Lets Respect box, the bulletin and the website.
 
Please take some time to complete the following evaluation form. It should take less than five minutes to complete, if you are unable to email either fax or let us know if you need to post.
 
Your response is essential and is important as it will determine the next steps for Lets Respect.

This survey is anonymous.
The record kept of your survey responses does not contain any identifying information about you unless a specific question in the survey has asked for this. If you have responded to a survey that used an identifying token to allow you to access the survey, you can rest assured that the identifying token is not kept with your responses. It is managed in a separate database, and will only be updated to indicate that you have (or haven't) completed this survey. There is no way of matching identification tokens with survey responses in this survey.
